Student/Candidate Judge Annual Activities Report

All Student and Candidate Judges should complete the following form by December 31 annually. All blocks should be completed or contain “NA”.

4. Please list the Year and Seminar location where you were accepted as a Student Judge.

5. Please list the Year and Seminar location where you were accepted as a Candidate Judge (successful completion of candidate exam).

6. Please list your current koi club membership(s).

7. Please list club officer position(s) held in the past three years.

8. Please list koi show judging assignments you have completed this year. Example: Name of Show, Location, Dates, Name of Head Judge

9. Please list shows where you exhibited your koi this year. Example: Dates, Name of Show

10. We are also interested in your participation and volunteer activities beyond judging show assignments this year. Please list all activities at shows (show chair, benching, fish handling, set up, etc.), seminars or presentations at koi events where you actively participated beyond being an attendee. Also list any articles written for publication, other special projects or presentations requested by the AKJA Judges committee. (Do not list attendance at local club monthly meetings unless you gave a koi presentation) Example: Date, Event, Brief Activity Description
