All Student and Candidate Judges should complete the following form by December 31 annually. All blocks should be completed or contain “NA”.

Please list all activities at shows (show chair, benching, fish handling, set up, etc.), seminars or presentations at koi events where you actively participated beyond being an attendee. Also list any articles written for publication, other special projects or presentations requested by the AKJA Judges committee.
